The two awards, organised by the Season Ticket Holders Association, are voted for by the 7,500 club season ticket holders.
This will be the first year of the Doug Wadley Trophy, which will be awarded to the Player of the Year.
The trophy is named after Doug Wadley, who played for the team and was a leading administrator over many years, finishing as Club secretary, shortly before his sad death at the end of 2003.
Season ticket holders can complete their voting slips in their books and hand them in at the ticket office.
There will be voting boxes in the main bar and members bar at the Leeds game on Saturday 9 April. Ballot Boxes will be available near entrances one, two, three and five. There will also be ballot boxes available in the Sports Bar and Members bar .
The awards will be presented before kick off at the final home game of the Premiership season against Saracens on Saturday 30 April.
